How Do I Order a Model?

These are the basic steps for ordering a ProtoMED model.

1. Contact ProtoMED to discuss your modeling needs. ProtoMED will provide a quotation for pricing and delivery of the model.

2. Determine who will reimburse ProtoMED for the cost of the model. ProtoMED will assist in communications with the insurance provider, hospital or patient, as needed. ProtoMED will need written documentation from the reimburser. This can be in the form of a letter stating prior authorization from an insurance provider or a purchase order from the hospital. For patient pay, the patient must pay by check or Visa or MasterCard before ProtoMED will ship the model.

3. Order a CT scan per the ProtoMED scan protocol. ProtoMED will provide assistance in finding a compatible scan site and communicating with the CT department about the protocol as needed.

4. Send the CT scan image data to ProtoMED via overnight delivery.

5. ProtoMED will create 3D reconstructions of the CT data. Eleven different views of the 3D reconstructions will be emailed to you for your approval.

6. ProtoMED will ship the model via overnight delivery. Typically, ProtoMED will provide the model within 3-5 working days upon receipt of good CT data and a written commitment to reimbursement.
